This assignment is a patient scenario case study: (Patient with cancer). It is a level 6 critique assignment that has 4 learning outcomes that must be well explained with pieces of evidence. It consists of:
– An evidence-based discussion of the epidemiology, or study of the incidence, distribution, patterns, and determinants of cancer ( with particular reference to colorectal cancer)and the impact and implications of this on Sarah and the wider society. This can include discussing the complication risks, impact to wider society like the cost to NHS e.g. hospital stays alcohol, and hereditary factors for cancer.
This evidence-based discussion will incorporate an examination of demographical factors related to the case study and how this has influenced health, illness, and wellbeing (LO1).
– Consideration of a specific example of a selected clinical investigation or assessment from the chosen case study. Here, I want to talk about the Breathing aspect of A-E assessment in the case study i.e. explanation of RR and o2 and their physiology. Talk about how this deterioration will be managed using a 15l non-rebreathe mask and monitor Saturation and a possible repeat of ABG (Arterial Blood Gases).
Talk about how the chemotherapy Sarah was on, caused neutropenia which caused chest infection that led to sepsis which brought about the low o2 and a high RR of 26. Please use physiology to describe this.
You will need to critically and analytically explore the chosen element and provide an evidence-based rationale for the recommendations for care. Refer to a range of relevant biological, physiological, pathophysiological, pharmacological, and nursing care knowledge, to evaluate the care and management of complex health needs. (LO2 and LO3). Talk about managing low Hb through blood transfusion.
– An evaluation of your role, as a registered nurse, in the provision of care to the chosen case study, which will centre around an appraisal of communication skills pertinent to the case study, considering interdisciplinary and cross-setting working (LO4). Here, I will use the SPIKES model of communication to discuss Sarah’s worsening condition and breaking of Bad news to the family working with the interdisciplinary team. I will talk about how to use my time and discuss how to handle her agitation.
Working with the interdisciplinary team, who will I tell when she is deteriorating? what will be my role and whose responsibility will it be to transfer her to ITU? what risk is involved in transferring her, when does she get transferred to the hospice? Am I involving the palliative care team? Has she got a Package of Care and how do I organise her care? These are all to be discussed when talking about her ongoing care as a registered nurse.
NOTE: Real terminologies to be used in all the physiology aspects of the assignment. It is a level 6 assignment that has to be critically written following the 4 learning outcomes with recent pieces of evidence,
Please see attached assignment guideline, marking criteria, and the patient scenario for more details.
Please link the progression of Sarah’s deterioration through the different stages as outlined in the case study.
Please add statistics and resources should not be older than 2013
